Genuine LiftMaster K41-0072-000 relay adapter board — the relay output board shared by all four of LiftMaster’s heavy-duty and industrial DC operators: the HDSW24UL, HDSL24UL, IHSL24UL and INSL24UL. It carries the relay outputs the control board uses to switch external devices, and mounts beside the power board on the K41-0249-000 mounting plate.

Is the relay board actually the problem?

After fitting
Photograph the harness before removing the old board and reconnect the new one the same way. Re-test each relay-driven accessory before handing the gate back.
Fit surge protection at the same time. Storm damage is a leading cause of gate operator board failure. If this board died after a storm, the replacement is at the same risk unless the installation is protected.
