Genuine LiftMaster K001D8395MC Logic 5 logic board (printed K001D8395) — the single- and three-phase, current-sense-ready main board for every Logic 5.0 commercial door operator: the T, GT, GH, H, HJ, J, SD, GSD and APT lines in their L5 form (T501L5, GH501L5, J501L5 and so on). It replaces the earlier K001D8075-1 and K77-37986, and LiftMaster now supplies this one number for all voltages and both phases.
Logic 5 = separate logic board and power board. Open the electrical box: an L5 box has this logic board plus a power board (single-phase K001D8396-1, three-phase K001D8397, 575 V K001D8397-1). If there is one board with relays on it and no separate power board, the operator is Logic 3 or 4 and this is not the part.

Is the board actually the problem?
The logic board is the most over-ordered part on a commercial operator. Work down this list before you buy:

After fitting
Set the limits, program the remotes and the timer-to-close, and re-test the monitored entrapment protection. On three-phase units confirm the current sense harness is connected before the first run.
Fit surge protection when you fit the board. Lightning and utility surges are the most common reason a commercial operator board is replaced, and a new board on the same unprotected supply is at the same risk. The MOV kit on the electrical box is the first line; a proper surge arrester on the supply is the second.
