Genuine LiftMaster K001A6426-2 control board (also written K1A6426-2) — the main control board in the separate wall-mounted control box of the RSL12V / RSL12VH slide and RSW12V / RSW12VH swing gate operators, the 12 V chain-drive and linear-arm residential operators with a mechanical limit shaft. The same board serves all four.
V, not U. The RSL12V and RSW12V are not the RSL12U / RSW12U and share very little with them. The U models have the electronics built into the operator with an APS encoder; the V models have this board in a separate box. Read the model on the label and the number on the board.

Two boards, two jobs. This board runs the operator. Remotes are received by the K001A6636 RF board; if the remotes fail but the wired controls work, that board or the antenna is the suspect, not this one. The RSW12V also has a separate K001A6407-1 absolute encoder logic board.

Is the board actually the problem?
The logic board is the most over-ordered part on a commercial operator. Work down this list before you buy:

After fitting
Set the limits, learn the remotes through the RF board, and re-test the entrapment reversal and any loops or eyes before returning the gate to service.
Fit surge protection when you fit the board. Lightning and utility surges are the most common reason a commercial operator board is replaced, and a new board on the same unprotected supply is at the same risk. The MOV kit on the electrical box is the first line; a proper surge arrester on the supply is the second.
