Genuine LiftMaster K001A6039 control board — the board for the original LA400 linear actuator swing gate operator, the version with the metal control box and the green board. It does not fit the later LA400DC (plastic box) or the LA400U/UL.
Is your board green in a metal box? Then this is it. If the board is in a plastic box, read its number: K1D8052-1CC means an LA400DC; K1D8388-1CC means an LA400U or LA400UL. The three boxes share almost nothing; the arm parts are the same on all of them.

Is the board actually the problem?

After fitting
Photograph the harness connections before removing the old board and reconnect the new one the same way. Then set the limits and force, relearn every remote, re-enter timer, loop and accessory settings, and re-test every entrapment protection device to UL325 before returning the gate to service.
Fit surge protection at the same time. Storm damage is a leading cause of gate operator board failure. If this board died after a storm, the replacement is at the same risk unless the installation is protected.
